New Patients Information
First Visit & Scheduling
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
Important:
All patients under 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Pre-operative consultation and physical examination are mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink for at least six hours prior to your surgery. You will also need an adult to drive you home.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. They may be emailed to us if they are in a digital format. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
Scheduling
The Capital Center® for Surgery is open Monday through Thursday from 7:00 AM until 4:00 PM and Friday 7:00 AM until 3:00 PM. We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day.
We try our best to stay on schedule to minimize your waiting. Due to the fact that the Capital Center® for Oral and Maxillofacial Surgery provides surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

Review Your Treatment Plan
After your consultation in our office, one of our treatment coordinators will present your treatment estimate to you in person. If you are unable to wait, they can email the estimate to you instead. If you have any questions, please call our office at 202-386-7100, or you can reach our treatment coordinators via email at treatments@ccomfs.com.

Financial Information & Policies
Consultations
All patients must have a consultation prior to any treatment carried out at Capital Center®. This is applicable to new patients, referrals, or other means of special registration.
Cancellations
We charge a cancellation fee for patients who fail to come to their surgical appointments with less than 48 hrs notice. If you must change your appointment, we ask that you contact our office 48 hrs before to reschedule your appointment. We attempt to practice at the highest level and if patients do not arrive on time for their appointment, there’s a significant inconvenience for our other patients. You will be charged $50 per 15 mins of appointed time if you fail to arrive on time for your scheduled appointment.
Payment
At Capital Center® we accept all major credit cards. We also accept Care Credit and Checks.
Valuables
We are not responsible for lost or stolen items and encourage you to leave valuables at home.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Insurance
We make every effort to provide you with the finest surgical care and the most convenient financial options. Please bring your insurance information with you to your appointment. Your insurance policy is a contract between you and your employer or other entity; therefore, it is your responsibility to understand your policy limits, deductibles, scope of coverage, rules, and restrictions. We work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.
To accomplish this goal, we will gladly submit the necessary and appropriate forms to your insurance carrier at your convenience. If you have any problems or questions, please ask our staff. They are well-informed and up-to-date. Your policy provider may also assist you with specific questions regarding coverage.

Dr. Emery is out-of-network with ALL insurances. We will still submit claims to your insurance as a courtesy, so all reimbursement will come directly to you.
