If you are a New Patient to Capital Center ® you can Request Access to our Patient Portal and save time filling out all Required Medical Forms before you come to our practice or Review Medical Information 24/7. In order to request access please fill out the form below. After you’ve submitted your request, one of our Patient Coordinators will create your user and send you an email with your USER and PASSWORD. All connections are encrypted and HIPPA Compliant.
