A welcome email will be sent you with details on how to log in to our Patient Portal. According to what your condition is, our doctor will request specific health history and medications you may have previously taken. All records and information disclosed between you and doctors are protected under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

CONFIRM YOUR CONSULTATION

You will receive an email with details of your virtual/phone consultation. A link is included where you need to confirm your appointment after you’ve checked all info is correct.  If you can’t complete the appointment confirmation via email, your appointment and payment will be refunded.

EXPLAINED: EMERGENCY CONSULTATION

Emergency consultations were created for new patients who do not have an established relationship with Capital Center®. To establish this relationship, establish a profile in our patient portal, and gain access to our Doctor-On-Call please complete the emergency consultation request form above.

Please note our Patient Portal is a secure system that complies with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) requirements related to collecting and sharing of medical information. If you believe your condition is a life-threatening emergency, please disregard these instructions and call 911 immediately.

INSIDE OF AN EMERGENCY CONSULTATION

VIRTUAL / PHONE SESSION

Once you complete the emergency admission request form, you will select an emergency consultation time-slot from our online scheduling system. The emergency consultation will be carried out via Phone Call or Video Call (you pick). Video calls are held over our HIPPA-Compliant Zoom meeting room. The virtual session is automatically created once you select your time and date.

DIAGNOSIS

During your emergency consultation, our doctor-on-call will ask you questions on a number of different topics and perform a medical assessment to determine whether a diagnosis is possible. During this process, you will have the opportunity to ask questions and chat more with your doctor. There are some instances in which a diagnosis will not be possible without further in-person evaluation – if this is the case your doctor-on-call will advise you about the next steps. 

TREATMENT OPTIONS

Once your diagnosis has been determined, your doctor will explain your treatment options based on their thorough medical assessment. In some cases, the doctor may be able to call in a prescription or explain your treatment plan over the phone, in other instances you may be required to come in for further evaluation. Every case is different so treatment plans will vary based on your symptoms and evaluation.

PRESCRIPTION
TO PHARMACY

In the event that our doctor-on-call determines it is possible to treat your condition with a simple prescription, or a prescription is needed until you can come in-person, our staff can call in your prescription to your pharmacy of preference. Make sure you have the name, address, and phone number of your favorite pharmacy on-hand prior to your consultation. 

LIFE OR DEATH
EMERGENCY ADMISSION

If it is determined you require immediate hospitalization, your doctor will explain the next steps and help you get the care you need. 

ELECTRONIC
HEALTH RECORDS

All your prescriptions, health forms, medications, X-rays, treatment plans, and evaluation histories will be recorded in your electronic health record and will be available for you to access through the patient portal.
